Everything you need to know about your rights and records as a client
In accordance with Texas law (HB 4224, effective September 1, 2025), the following information is provided for all clients receiving behavioral health services.
Requesting Your Health Care Records
Clients have the right to access and obtain copies of their health care records.
To request your records, please submit a written request via email to jenn@jennziglerlpc.com
Please include your full name, date of birth, and a description of the records requested. Additional authorization may be required to process your request.

Filing a Consumer Complaint
If you have concerns about services provided, you have the right to file a complaint.
Complaints may be submitted to the
Texas Behavioral Health Executive Council
You may also file a complaint under Texas Health & Safety Code §181.103 regarding privacy or records concerns.
